Community Clinics

Community clinics offer wound care, post-surgical care, IV treatment and other specialized nursing services, and even the simple act of getting to one of our eight convenient community clinics is good for you.

Our community clinics are by appointment only, offering quick and easy access to nursing care for patients recovering from illnesses, accidents and surgeries. After an initial referral from your care coordinator, you can book appointments to suit your schedule.

Community clinics keep you out of hospital and help you recover at home, in more ways than one. Patients who make the effort to visit one of our clinics find the act of getting out of the house an important part of their healing process. We know it's not easy, but getting active again is an important part of your recovery process. If you are mobile enough to get around, you'll recover faster as you begin to take control of your care, get out into the community, and resume your normal routine.

Conveniently located close to hospitals and on bus routes where possible, our community clinics are open 8 a.m. - 8 p.m., 365 days a year. You will find community clinics in Alliston, Newmarket, Markham, Richmond Hill, Vaughan and North Toronto (Fairview Health Centre, Humber Clinics, North York Judy Dan Wound Clinic).

Community clinics save you time and reduces the need to visit the hospital department for routine nursing services. Annually, we provide high quality, specialized nursing care to over 10,000 patients as a cost-effective, innovative way to provide better care for patients.
