The CCAC e-health platform provides us with access to common information and the ability to support strong connections and effective communication with our partners across the health-care system, for example hospitals, primary care and community support services. This helps us better respond to people's needs and create a smooth care experience for you and your family.

MyChart™is a new online initiative in partnership with Sunnybrook Health Sciences Centre to give patients electronic access to their medical records. MyChart™ puts patients in control of their health care data and gives them the option to share it with anybody in their network of care.

Any interested Central CCAC patient can register for MyChart™ by calling 1-888-470-2222 ext. 5050. Information from your Central CCAC patient record is loaded automatically.

MyChart™is a secure site using the same encryption technology used by major banks worldwide.
