Anyone can call to find out about home and community care support services. We have a responsive team of staff who are able to answer your questions and help you access the care you need.

2021 Update: Effective April 1, 2021, our organizational name is Home and Community Care Support Services Central. Previously, you may have known us as either the Central Community Care Access Centre (CCAC) or Central Local Health Integration Network (LHIN).
While our name has changed, our services remain the same. There will be no interruption to your care. You will have the same care providers, and you can use the same phone number to get in touch with us.
As always, our priority is timely access to safe quality care for patients, their caregivers and families.
