Anyone can make a referral to us on your behalf: a family doctor, friend, family member, even you, yourself. If you think that you or someone you care about may benefit from our services, we encourage you to get in touch with us.

Just one call
Your first step is to call our main phone number 310-2222 which will automatically direct your call to us (no area code required) – or you can call us directly. Let them know you are making a referral for services.
If your health-care provider has already made the referral for you, you can expect a phone call from us once the referral has been processed.
Our helpful staff may ask some questions to better understand your needs. Then, you will either be connected with a Care Coordinator, or we will refer you to the program or service that may be right for your needs.

What is a Care Coordinator?
Care Coordinators are regulated health professionals, with expertise in nursing, social work, occupational therapy, physiotherapy or speech therapy. They work directly with patients in hospitals, doctor's offices, communities, schools and in patients' homes.
