Living at home and as independently as possible is often a high priority for patients and their families. In many cases this can be accomplished successfully with the right health care supports in place, including use of Integrated Care Clinics, specialized services and school health support services. However, depending on your needs you may also find other community services helpful including:
- Meal delivery and dining programs
- Homemaking and home help
- Transportation services
- Community dining
- Friendly visits
While Home and Community Care Support Services Central West doesn't provide these services directly, we can connect you with organizations that offer services such as those above. Ask your Care Coordinator about how these and other services, which can help reduce the stresses of daily living for both patients and caregivers, might be of benefit to you.
