Living at home is often a high priority for patients and their families. While in many cases this can be accomplished successfully with the right health care supports in place, depending on your needs you may also find other community services helpful. These might include:
- Meal delivery and dining programs
- Homemaking and home help
- Transportation services
- Community dining
- Friendly visits
While the CCAC doesn't provide these services directly, we can connect you with organizations that offer services such as those above. Ask your Care Coordinator about how these and other options might be of benefit to you – these services can help reduce the stresses of daily living for both the individual and the caregiver.
