Care in Your Community

​Living at home is often a high priority for patients and their families. While in many cases this can be accomplished successfully with the right health care supports in place, depending on your needs you may also find other community services helpful.  These might include:

  • Meal delivery and dining programs
  • Homemaking and home help
  • Transportation services
  • Community dining
  • Friendly visits

While the CCAC doesn't provide these services directly, we can connect you with organizations that offer services such as those above.  Ask your Care Coordinator about how these and other options might be of benefit to you – these services can help reduce the stresses of daily living for both the individual and the caregiver.

Community clinics provide services that address specific health care needs, such as IV therapy, wound care, or rehabilitation therapy. Visits are arranged byus and are by appointment only. Community clinics are staffed by specially trained nurses and are not the same as visiting a walk-in medical clinic.

Specialized services are offered by community health care providers for people with specific conditions or care needs, including:

  • Acquired brain injury
  • Mental health and addictions issues
  • Convalescent care
  • Alzheimer's disease and related dementias
  • Stroke recovery
