What is the difference between an integrated care clinic and a walk-in clinic?
These clinics are not open to the general public. You must be assessed and referred to the clinic by Home and Community Care Support Services Central West
- Visits to an Integrated Care Clinic are by appointment only. Walk-ins are not accepted.
Integrated Care Clinics are staffed by qualified, experienced nursing teams. Primary care access is not available.
Patients must be covered by OHIP.
WHAT TYpes of services can I expect to receive at the clinic?
Staffed with qualified teams of health care professionals, clinics provide a variety of specialized nursing services to patients of all ages. Depending upon the specific clinic, these services may include:
- Wound care
- Drain care
- Injections
- Central line maintenance
- Catheter care
- Care coordination and planning
- Diabetes management counselling and education
| - Enterostomal therapy/ostomy care and education (ET nurse available to see patients at the clinic once per week)
- IV therapy and IV access maintenance
- Chemotherapy disconnects
- Lymphedema care
- Education on self-management, incuding but not limited to diabetes and injection
