Integrated Care Clinics

​​​​​​​​Central West LHIN Integrated Care Clinics provide Home and Community Care patients who need nursing services, with improved access to high-quality, patient-centred care. Staffed with qualified teams of health care professionals who specialize in a variety of nursing services such as wound care and intravenous (IV) therapy, these clinics provide benefits to patients of all ages.​

  • Availability of supplies so you don’t have to worry about storage, delivery or pickup.

What is the difference between an integrated care clinic and a walk-in clinic?​

  • These clinics are not open to the general public. You must be assessed and referred to the clinic by Central West LHIN Home & Community Care.​
  • Visits to an Integrated Care Clinic are by appointment only. Walk-ins are not accepted.
  • Integrated Care Clinics are staffed by qualified, experienced nursing teams. Primary care access is not available.
  • Patients must be covered by OHIP.​

WHAT TYpes of services can I expect to receive at the clinic?

Staffed with qualified teams of health care professionals, clinics provide a variety of specialized nursing services to patients of all ages. Depending upon the specific clinic, these services may include:
  • Wound care
  • Drain care
  • Injections
  • Central line maintenance
  • Catheter care
  • Care coordination and planning
  • Diabetes management counselling and education​
  • Enterostomal therapy/ostomy care and education (ET nurse available to see patients at the clinic once per week)
  • IV therapy and IV access maintenance
  • Chemotherapy disconnects
  • Lymphedema care
  • Education on self-management, incuding but not limited to diabetes and injection​
