Finding a Family Doctor

Health Care Connect LogoIf you need a family doctor in Ontario, Health Care Connect can help.

A program developed by the Ministry of Health and Long-Term Care, Health Care Connect is a publicly-accessible website and phone service that helps people through the process of finding a family doctor or nurse practitioner who is accepting new patients.

To access the service, you must be currently without a family doctor or nurse practitioner, and you must have a valid Ontario Health Card. The Health Care Connect website outlines their simple registration process, or you can call
1-800-445-1822 for more information.

CCACs play an integral role in Health Care Connect. After you have joined Health Care Connect, a nurse, called a Care Connector, will help you find a health care provider in your area. Your Care Connector will be your main point of contact and will provide you with more information by mail after your registration is complete.
