Central West CCAC, Headwaters Health Care Centre (Headwaters) and William Osler Health System (Osler) are pleased to respond to the Ministry of Health and Long-Term Care's Patients First: A Proposal to Strengthen Patient-Centred Health Care in Ontario.
The Central West CCAC, Headwaters, and Osler are partners in one of the fastest-growing and most culturally and geographically diverse communities in Ontario.
The partners have worked together over several years to improve patient experience in the region. This involved strengthening the partnership at the governance, leadership and clinical delivery levels; navigating structural, financial, and cultural integration challenges; and above all, putting patients front and centre.
The purpose of this response document is to outline learnings to help inform provincial planning around integrated health care service delivery, in response to questions posed in the Patients First consultation document under the heading of Integration & Equity.
