The role of the Community Care Coordinator

Aline Payne has been working in the Peel area for 27 years and has been with the Central West CCAC since before it was known as Central West.

She started as a community physiotherapist with the Region of Peel Homecare in 1990, then transitioned to working as a Case Manager at the Region of Peel CCAC, which became part of the Central West CCAC in 2007.

"My role is to assess patients in the community and coordinate their services. I also do assessments for patients applying to assisted living, day programs and long-term care, and provide patients and their families with links to community resources. I am aligned with specific family doctors and meet with them regularly to discuss our patients’ service and treatment plan," she says. "The most rewarding part of my job is to know that I have been able to help someone in their journey in health care. Our health care system is complex, and our patients and their families appreciate it when you help them navigate through it."
