Nomination Deadline Extended to Honour Heroes!

Through their caring, compassion and support, silent 'heroes' in the Central West community are improving the lives of others every day – and the Central West Community Care Access Centre (CCAC) wants to acknowledge these unsung home care heroes. 


Nominations for Central West's third Heroes in the Home awards have been extended and will be accepted until October 14th.  The program acknowledges all forms of caregivers: family members, friends, volunteers, personal support workers, care coordinators, nurses, therapists and other health care professionals.  Past recipients have included neighbours who shovel walkways for seniors in their community, parents caring for children with complex medical needs, and personal support workers who provide much-needed assistance to help others maintain their independence.


There is no limit to the number of caregiving 'Heroes' someone can recognize. Each nominee will receive a certificate of recognition and nominators, nominees and their families will be invited to attend a celebration in their honour this fall.

About the Central West CCAC

The Central West Community Care Access Centre (CCAC) helps ensure people have the health care and support they need to remain safe and independent for as long as possible. We are privileged to help people come home from hospital, live independently, and, when appropriate, transition into long-term care. Our care coordinators are regulated health professionals – nurses, physiotherapists, and others – with additional, specialized training in coordinating care across the health system. Last year, we served more than 35,000 patients at home, in school, and through the three hospitals located in our community. The Central West CCAC is funded by the Central West Local Health Integration Network (LHIN).
