Care Coordinators

Care Coordinators are the heart and soul of home and community care. They not only manage your initial contact and assessment when you are new to us, they also ensure that you receive, and continue to receive, the ongoing care you need when you need it.

Who are Care Coordinators?

Our Care Coordinators are regulatedA Care Coordinator and client pose for the camera health professionals with expertise in nursing, social work, occupational therapy, physiotherapy or speech therapy, who work directly with patients in hospitals, doctor's offices, communities, schools and in patients' homes.

What do Care Coordinators do?

Care Coordinators are connected to every part of the health-care system and can serve as your single point of contact in obtaining services and information.

Care Coordinators use their professional health knowledge and assessment skills to understand your individual needs, making recommendations based on your needs and goals.

We work hand in hand with you and your family to find out what care you need and then work with you to develop a care plan that is right for you – whether it's nursing care, meal delivery, a day program, or help to find a family doctor.

How do I connect with a Care Coordinator?

Care Coordinators are assigned to you once you become a patient of ours. Referrals can be made by a health-care professional, a family member or friend, or even by you, yourself. Simply get in touch with us and we'll help you get started.
