If you, or someone you care about, needs health care services at home, at school or in the community, or if you are considering supported living programs or long-term care options, home and community care at the Champlain Local Health Integration Network can help.
Champlain LHIN works with people of all ages to ensure they can make informed choices about their care, when and where they need it.
We also have useful information about local community support service agencies, and can link people to these providers to arrange services.
You can explore your care and support options through our website, or contact us at 310-2222.
Recent Changes Related to Home and Community Care
On May 24, 2017, home and community care services and staff transferred from Community Care Access Centres (CCACs) to Local Health Integration Networks. Home and community care services are now provided through Ontario’s 14 Local Health Integration Networks (LHINs). LHINs plan, integrate and fund local health care. We also deliver and coordinate home and community care.
As part of the Government of Ontario’s Patients First: Action Plan for Health Care, this health system change was seamless in order to ensure uninterrupted care and support for patients and home care clients.
