Cross-sector collaboration is yielding some exciting successes. Home First is another example of how we are leading some important shifts in our health system's ability to ensure the right care, at the right time, in the right place. The philosophy is focused on keeping high-needs seniors safe in their homes for as long as possible with home care and other community services. Working with the Local Health Integration Network and our hospital partners, Home First has been rolled out successfully in a number of hospitals throughout the Champlain region:
- Before Home First, 55% of placements to Long-Term Care Homes were from hospitals. Today, 33.2% of placements are from hospitals and 66.8% of placements are from the community.
- In 2013, an average of 3406 days hospital days avoided.
- 86% of Home First patients remain in the community after 90 days.
- Estimated annual saving: $10 million.
