Patient First: A Proposal to Strengthen Patient-Centered Health Care

On behalf of the Champlain LHIN Board I am pleased to invite you (as a client/user of the system) to participate in a teleconference round table on the discussion paper:  Patient First:  A Proposal to Strengthen Patient-Centered Health Care in Ontario  (available at this link: http://www.health.gov.on.ca/en/news/bulletin/2015/hb_20151217.aspx ).  The forum will take place on Wednesday, February 24 from 1:00 to 2:30 p.m.

The Champlain LHIN is presently involved in a series of engagement at various levels in our region to collect feedback from our community. As part of this community engagement, the Champlain LHIN Board would like to engage with members of the public.  We are sharing this invitation with health service providers so they will in turn share with their clients.

Participation is limited and we ask that you please register your participation with Sylvie.Bleau@LHINs.on.ca by February 17, 2016 (see registration details below).  A confirmation with further details will be sent to you as we get closer to the date.
