Innovative partnership offers patients flexibility and a proactive role in their care
The Champlain CCAC, Saint Elizabeth and Rideau Valley Health Services (RVHS), have partnered to open a new community care nursing clinic at the Rideau Valley Health Centre in Barrhaven.
Community nursing clinics are proven best practice care and promote improved healing and well-being for patients.
Community nursing clinics provide a broad range of wound care, intravenous infusions services and more to people who require nursing treatment and education in self-care.
They also maximize health care funding and foster a sustainable system that ensures we continue to provide health services to as many people as possible in the community while freeing up more nurse time for high-care need, homebound individuals.
Clients that visit nursing clinics benefit from:
- A scheduled appointment – no waiting at home or in the ER
- Flexibility - clinics are open 7 days/week and offer a variety of appointment times
"The nurses at the clinic are the best! I have received the most efficient TLC (tender love and care)," said Marcel Chartrand, patient at the Barrhaven clinic.
With 19 clinics conveniently located across Champlain, CCAC patients will receive care close to home. If necessary, patients will continue to receive nursing at home if not appropriate for a clinic. Visits are arranged by your care coordinator as part of your assessed care plan needs. They are by appointment only. CCAC clinics are not the same as visiting a walk-in medical clinic.
"Expanding services and providing care within the community is why we were excited to partner with Saint Elizabeth and Champlain CCAC" notes Catherine Van Vliet, RVHS Administrator and Executive VP Integration and Primary Health, Kemptville District Hospital. "Receiving care in a clinic setting allows our patients to take a proactive role in their own recovery. Patients can arrange for required care at a time and location most convenient for them and design their care schedule to best suit their personal needs. It is about the patients and what works for them."
