Care Report

​I’m pleased to bring you the July issue of the CCAC Care Report. It’s hard to believe that I have been with the Champlain CCAC for a year – and what a year it has been! Overall, it has been an inspiring and overwhelmingly positive experience. I want to thank you for your partnership and commitment to providing the best care possible to patients and their families across the region. 

As you will note in the Program Capacity snapshot below, we are seeing unprecedented demand for home and community care services. Indeed, we averaged 21,500 patients per month in April and May, which is a 3.1% increase compared to the fourth quarter of fiscal 2015/16. To ensure we operate within our resource allocations, we have introduced some measures for lower needs patients – partial waitlisting of PSS and therapies (except physiotherapy). 

We are monitoring the situation closely and doing everything possible to minimize impact to patients, particularly very high need and Hospital to Home patients. Hospital to Home patients are not impacted.

Advancing Integration

Please take a moment to read our Annual Report to the Community to learn about our progress over the past year and some of the new initiatives underway. Allow me to highlight a few examples:
  • Our new IMPACTT Innovation Centre is evaluating and recommending new home health technologies to improve support to patients and their caregivers – and help them stay at home for as long as possible. Learn more about the IMPACTT Centre and how to book a tour! 
  • A hospital e-notification system established with Kemptville District Hospital, Renfrew Victoria and Arnprior Regional Health is enhancing information sharing among members of the care team – a win for patients and providers!
  • We have enhanced our focus on Patient and Caregiver Engagement with the addition of a new Patient Advisor Program, which aims to ensure that even more patients and families across the region have the opportunity to shape our programs and services.
