I'm pleased to share information on the Champlain CCAC's current capacity and progress related to ongoing improvement activities that will support our growing patient volumes and unprecedented acuity levels. Each month, I will be sending you a brief status report on key metrics to highlight our capacity, and progress against our new action plan:
Maintaining Increasingly Complex Patients at Home.
Fiscal 2015/16 saw a continued rise in demand and patient complexity. We served 58,900 patients last year. Referrals increased by 3.8% in fiscal 2015/16.
We were able to release our waitlists and assume new patients. Last year we had over 1,939 personal support services patients on waitlists, and as of March 31, 2016, we had 63. Partnerships were essential in helping us to achieve this considerable feat.
I am committed to keeping you informed of our progress and priorities as the system transformation continues to unfold. Collaboration with our many local system partners will ensure we are well positioned to meet the increasing needs of people across Champlain—and support them in remaining at home and in the community for as long as possible.
