I’m pleased to bring you the September issue of the CCAC Care Report – your source for information on key home and community care initiatives and current program capacity. As noted in the Program Capacity snapshot
, we continue to see an unprecedented increase in referral volume. Home care referrals are up 4.9% in April through August compared to the same period last year.
And the number of higher acuity patients has increased by 15.9% since last year.
The good news is we are supporting more people than ever before at home and in the community – where they prefer to be. However, we have now reached almost 112% in overall patient capacity with some programs such as Stay at Home exceeding at 132%.
Given our obligation to operate within our allocated resources, we must implement more stringent budget measures for the next two-month period. High need patients will continue to receive services as usual.
We are working to minimize patient flow issues – we have an exception process in place to ensure continuity as much as possible. Hospital to Home services will continue.
In the meantime, we will monitor trends carefully and assume more patients as soon as capacity becomes available. Thank you for your ongoing support and partnership.
