​The About Us brochure​ is designed to provide an overview of the home and community care services available through the ESC LHIN. 

palliative care services

The ESC LHIN connects, supports, and cares for patients who have a life-limiting illness. Our Palliative Care Services brochure provides an overview of the services available through the ESC LHIN in a patient’s home and in the community. This brochure also provides information on how we support family members during their loved one’s care journey.



mental health and addictions nursing

Our MHAN nurses work throughout school boards across the Erie St. Clair region. The MHAN brochure outlines the services we provide to students with mental health and addictions concerns, including how to refer someone to the program.




convalescent care

The Convalescent Care brochure provides a summary of the convalescent care program and identifies the services available through the ESC LHIN and our community partners. This program is designed for patients following a surgery or serious illness who require specific medical and therapeutic service​s in a supportive environment.



Patient Advisors

The Patient Advisors brochure provides an overview of the opportunities available for patients, their families and/or caregivers to volunteer at the ESC LHIN by providing feedback to our organization through their experiences.



​Receiving IV care in clinic

The ESC LHIN provides IV treatments in our clinics for eligible patients. This handout​ provides information to help patients understand the roles and responsibilities involved in their care.



The ESC LHIN welcomes feedback from our patients and their families. The Feedback, Complaints, and Appeals document outlines how we manage this feedback and explains why it’s so important to us.



Our Long-Term Care Home Information Package ​is designed to help those exploring long-term care options. We also have lists of long-term care homes in Chatham-KentSarnia-Lambton and Windsor-Essex.​​
