ESC CCAC Strategic Plan 2014/2015 - 2016/2017
Partners in Care: Erie St. Clair CCAC Strategic Plan 2014/2015 – 2016/2017 is the work of a community. Developed with input from hundreds of patients, caregivers, community partners and CCAC staff members, this plan centres upon three strategic directions: Delivery of Patient and Family Centred Care (PFCC), Reducing the Impact of Chronic Disease and Being a Partner of Choice in Enhanced Care Coordination. These three strategic directions will be achieved with the help of our three strategic enablers: Value for Investment, Effective Use of Technology and Being an Employer of Choice.
Each of our strategic directions and strategic enablers include specific goals and objectives for the next three years. Our progress in meeting these goals and objectives is updated regularly on the Strategic Deliverables document.
Involving our Partners in Care – from patients to caregivers to health system partners – in our journey is integral to our strategic plan. Our strategic directions increase the role of the patient through initiatives like the Patient and Family Advisory Council (PFAC). As we continue this journey, we will be holding annual accountability sessions to receive feedback on how we are progressing in our journey.
