The Erie St. Clair Local Health Integration Network (LHIN) is honoured to be hosting Heroes in the Home 2019 this spring. This great event celebrates the dedication and selflessness of caregivers and highlights the extraordinary work of these individuals.
Who does the program recognize?
The Erie St. Clair LHIN Heroes in the Home caregiver recognition program will honour both informal and formal caregivers including family members, friends, loved ones, co‐workers, health professionals, and others whose kindness and commitment allow people to live their lives in their communities, despite the limitations of age, illness, or disability.
How do I nominate someone?
There are two ways to nominate a Hero in the Home:
1. Complete the 2019 Heroes in the Home Nomination form online
2. Download, print, and mail the 2019 Heroes in the Home Nomination Form

How does a caregiver find out they’ve been nominated?
Please notify your nominee in advance and let him/her know that they will receive a letter of congratulations. All nominees along with the individual who nominated the caregiver will be invited to attend a special awards ceremony. Nominees will be asked to RSVP and return a signed consent form.
What will nominees receive at the event?
Each hero will
receive a certificate, a pin, and a scroll with their caregiving story.
Will caregiver relief or transportation costs be covered?
Although we are unable to cover caregiver relief or travel costs, events will be held in various locations throughout our LHIN region. Attendees will be invited to attend the most convenient event for them. If someone is unable to attend, their certificate of appreciation will be sent to them following the event.
