CCAC Ready To Support New Hospices

​​Chatham and Leamington, ON – The Erie St. Clair Community Care Access Centre (CCAC) is committed to collaborating with and supporting the new Chatham-Kent Hospice and the Erie Shores Campus of the Hospice of Windsor-Essex.

Both new hospice locations will begin accepting patients shortly and those patient referrals will be provided by CCAC Care Coordinators.
 
“Our CCAC already has a wonderful relationship with hospices in Windsor and Sarnia, and with a greater demand for hospice services in the communities we serve, we look forward to begin working with the new locations in Chatham and Leamington, further ensuring patients are receiving care in their preferred setting,” said Patient Services Manager Cindy Stokes.
 
Over the last eight years, the CCAC has been providing end-of-life services in the home through its Palliative Care Consultation Team (PCCT), which now supports nearly 1,300 patients across Erie St. Clair.
 
The PCCT consists of eight Nurse Practitioners who provide palliative care during the last six to 12 months of life for patients living with a life threatening illness. The team also includes occupational therapists, social workers, resource nurses and physician leads that meet regularly and work closely with hospice and hospital partners.
 
“This collaboration has really helped us support patients to be cared for in the setting of their choice at end of life, whether it’s at home or in hospice,” said Stokes. “PCCT is able to anticipate outcomes related to life threatening illnesses that patients are living with and ensure proper treatment is in place. In development of the hospices, there has been increased palliative knowledge acquired by physicians and interdisciplinary team members. This helps reduce emergency room visits and hospital admissions.”
 
If a patient is no longer able to remain in their home, a CCAC Care Coordinator will make a referral for hospice care. The hospice then determines eligibility and collaborates with the Care Coordinator on admission and transition to hospice; the patient continues to have access to CCAC PCCT services if required.
