Anyone can call to find out about home and community care services. We have a responsive team of staff who are able to answer your questions and help you access the care you need.

There are a variety of supportive resources available on home and community care services, including a Guide to our Services, and a Support Guide for Caregivers.
As part of Ontario's Patients First: Action Plan for Health Care, all home care services and staff from the Erie St. Clair Community Care Access Centre (CCAC) transfered to the Erie St. Clair Local Health Integration Network (ESC LHIN) on June 21, 2017. This change looks to improve access to care, reduce wait times, and improve the patient experience.
Office locations remain the same. You can use the same phone numbers, email addresses, and websites to get in touch with your providers and arrange your care.
