CCACs are committed to safe, reliable and improved care at home and in our communities. Earlier this year, Ontario's Minister of Health and Long-Term Care, Dr. Eric Hoskins, unveiled a roadmap to strengthen and transform home and community care across the province (PDF). As a step towards improving care and consistency of service delivery province-wide, the Ministry of Health and Long-Term Care is seeking expert advice on a Levels of Care (LOC) Framework. The ministry's goal is "to ensure services and assessments are consistent across the province." Ontario's CCACs are uniquely positioned to provide valuable input based on their experience and expertise in home and community care.
In November 2015, we delivered a preliminary advice document — Establishing an Effective Levels of Care Framework for Home & Community Care in Ontario (PDF) — and we invite you to view our full paper.
We released a second advice paper, Approach for Developing the Levels of Care Framework (PDF), in December 2015. In it, we highlighted additional considerations for the ministry as it works to develop a Levels of Care Framework that ensures equitable access to home and community care across Ontario.
You can also read our research on how other countries and provinces assess care needs and deliver services to their populations.
