Reviewing ESC CCAC’s Strategic Plan

​​​Chatham, ON – Following a year of technological advancement at the Erie St. Clair Community Care Access Centre (CCAC), the organization is further exploring digital technology to host its annual Strategic Planning Accountability Session.

The CCAC is entering the final year of its three-year strategic plan and will provide updates to attendees via a live webcast on Friday, September 30, 2016 from 1:30pm until 3pm.

The Strategic Plan for 2014/15 to 2016/17 includes three directions – Patient and Family Centred Care, Reducing the Impact of Chronic Disease and Enhancing Care Coordination; and three enablers – Value for Investment, Effective Use of Technology and Being an Employer of Choice.

“We had a particularly strong year in terms of Patient and Family Centred Care as well as our use of technology to increase administrative efficiency and provide support in the home,” said CEO Lori Marshall. “This includes eShift nursing and personal support for end-of-life patients, Telehomecare for self-management of chronic disease, and eNotification and eReferral for communicating with our health sector and community support partners.”

This accountability session is open to everyone and will provide updates on the success, challenges and current state of the strategic plan deliverables. As well, participants will have the opportunity to provide feedback and interact through the use of digital tools.

“Our Strategic Plan is available on our website and our progress is consistently measured and updated,” said Marshall. “The input we gather during our annual review sessions helps us to ensure we are keeping on track with the needs of the communities we serve. We do make adjustments as needed based on what we hear during this collaborative process.”
