Office Location:  Bluewater Health

(Travel within Sarnia-Lambton is required)

NATURE OF THE POSITION

Through standardized assessments and working with other Behavioural Support Service Providers, the Care Coordinator will provide seamless, informed and coordinated transitions to meet the needs of older adults with cognitive impairments.  Working with caregivers, the Care Coordinator will focus their attention towards the ALC patient, in an acute care setting with a diagnosis of dementia and exhibiting responsive behaviours but may also see other mental health diagnoses with responsive behaviours in the acute care setting or current CCAC patients residing in the community of Sarnia-Lambton.  The Care Coordinator is considered part of the "core Health Services Providers" and will receive specialized Behavioural Support Ontario training.  Other duties may be assigned in order to meet the meet the needs of the organization.

STARTING DATE:              On or about 23 June 2014

POSITION STATUS:          Full-Time (M-F, 8:30 a.m. to 4:30 p.m.)

SALARY RANGE:              Commensurate with qualifications and

                                          experience in accordance with the Collective

                                          Agreement

QUALIFICATIONS:

  • A Baccalaureate degree from a recognized university in a health-related field such as Nursing (and/or a combination of nursing education, training and experience), Occupational Therapy, Physiotherapy, or Social Work (MSW required)
  • Must have two years of experience in providing direct clinical mental health services; and current registration with a college recognized by the RHPA who can determine capacity in accordance to the Health Consent Act (1996)
  • Excellent interpersonal communication skills
  • An oral/written proficiency with Canada's two official languages will be looked upon favourably.
  • Must have access to a vehicle and a valid Ontario Driver's License
  • Must be able to operate a computer
