JOB TITLE:  Temporary Full-Time Social Worker, Palliative Focus – Sarnia site  (Approximately 11 months)                  

LOCATION:  Erie St. Clair Community Care Access Centre; Travel for this position is required within Sarnia and Chatham/Kent counties

SUPERVISOR:  Patient Services Manager

Reporting to the Patient Services Manager, this position is responsible for the effective delivery of social work services to Palliative Care Consultation Team patients upon referral from Erie St. Clair Community Care Access Centre Care Coordinators and in accordance with professional college standards, Ministry guidelines and Erie St. Clair CCAC policies.  The objective of the services delivered by the Social Worker is to assist and support the patient with complex psycho/social needs at end of life.  The Social Worker will work with the patients and family throughout the end of life as part of the Palliative Care Consultative Team (PCCT).

The Social Worker will participate in events to promote CCAC objectives, activities, programs and services and will ensure that the Palliative Care Services, are provided in accordance with CCAC policies, relevant legislation, and professional disciplines.  Other duties as assigned, in order to meet the needs of the organization. Travel for this position is required within Sarnia/Lambton and Chatham/Kent counties.

NATURE & SCOPE:

  • Manages the delivery of coordinated, quality care Palliative Consultation services
  • Delivers Palliative Consultation services for the CCAC
  • Implements and maintains service standards, monitors and reviews caseload statistics and indicators; monitors service levels to ensure effective resource allocation
  • Identifies critical service delivery issues and refines services for the patient population as needed
  • Participates in the development, and manages the implementation and evaluation of new service delivery issues
  • Ensures palliative services are provided in accordance with CCAC polices, relevant legislation, and professional disciplines
  • Establishes and maintains effective relationships with service providers to deliver quality patient care
  • Participates in events to promote CCAC objectives, activities, programs and services
  • Establishes and maintains relationships with professional organizations in designated specialty functions to ensure quality of care is current
  • Works effectively with superiors and colleagues to deliver effective and efficient patient service delivery
  • Acts as a key resource to all CCAC staff regarding the Palliative Consultative Team
  • Provides advice and guidance in managing difficult or unusual cases
  • Identifies program needs and provides input to ongoing planning
  • Other duties as assigned

QUALIFICATIONS:

  • A member in good standing with the College of Social Work
  • Specialized knowledge of Palliative Services
  • Working knowledge of community resources and roles of health care professionals
  • Good knowledge of CCAC priorities, policies, practices and service standards
  • Good knowledge and understanding of legislation governing health care activities
  • Knowledge of direct care/case management models used in community health care organizations
  • Excellent written and oral communication skills. Proficiency in French language is an asset

Priority consideration will be given to candidates who possess knowledge of Canada's two official languages.
