The Erie St. Clair Community Care Access Centre (CCAC) is pleased to share our 2013-2014 Annual Report to the Community.
The Annual Report is a reflection on the 2013-2014 fiscal year at the Erie St. Clair CCAC as our organization worked collaboratively with community partners, patients and caregivers to deliver quality care to approximately 37,900 patients.
The 2013-2014 fiscal year was full of exciting new programs and initiatives at the Erie St. Clair CCAC, such as the introduction of our Rapid Response Nursing (RRN) and Mental Health and Addictions Nursing (MHAN) teams.
The past year also marked our first annual Heroes in the Home caregiver recognition awards which took place in November 2013. We recognized over 100 caregivers in the communities of Chatham-Kent, Sarnia-Lambton and Windsor-Essex. In addition, we opened our second community nursing clinic in Sarnia-Lambton in May 2013, introduced virtual tours of long-term care homes as well as reporting on long-term care wait time data.
