ESC CCAC focusing on Strategic Accountability

Chatham, ON – The Erie St. Clair Community Care Access Centre is gearing up to host its 2nd annual Strategic Planning Accountability Sessions across Windsor-Essex, Chatham-Kent and Sarnia-Lambton.

The first accountability sessions were held in September 2014 ahead of the development and approval of the ESC CCAC Strategic Plan 2014/15-2016/17. While the first sessions turned to health and community sector partners, patients and families, academics and policy makers to help identify key priorities for change, the upcoming sessions will discuss with these stakeholders the progress of the work being done in relation to this strategic plan.

"Participants will hear about the success, challenges and current state of these deliverables," said Caen Suni, Senior Director, Strategy & Performance Management. "We will have a broad discussion to ensure our planned activities are still relevant or if we need to somehow change course in light of new information or trends. As always, we are keen to hear from our partners if there are other ways that we can work together to further our common objectives."

The three Strategic Directions are: Patient and Family Centred Care, Reducing the Impact of Chronic Disease and Enhancing Care Coordination. The CCAC also has three Strategic Enablers: Value for Investment, Technology and Being an Employer of Choice. The sessions will involve table discussion between attendees and CCAC representatives based on these directions and enablers.

"Our full plan including our quarterly progress reports and key quality indicators are available on our website. Our results are transparent and we invite comment on these at any time," said Suni. "We are further committed to holding these accountability sessions on an annual basis in order to review and report progress on our plan, and make necessary adjustments to future goals based on the needs of the community. This commitment is aligned to our strategic planning direction and organizational philosophy that our patients, family members and partners must be part of our planning, quality improvement and day-to-day operations."

The Strategic Planning Accountability Sessions are scheduled for:

Chatham:
When: Thursday, September 3, 2015 from 1:00 pm – 4:00 pm
​​​Where: Erie St. Clair LHIN – 180 Riverview Drive - Lake Erie Room

Windsor:
When: Tuesday, September 8, 2015, from 1:00 pm – 4:00 pm
Where: Alzheimer Society of Windsor and Essex County – 2135 Richmond Street - Education Auditorium

Sarnia
When: Thursday, September 10, 2015 from 1:00 pm – 4:00 pm
Where: Marshall Gowland Manor – 749 Devine Street - Central Activity Area (CAA)
