When you or someone you care about becomes a patient a Care Coordinator will help in a variety of ways. Your Care Coordinator works with you to help you access the care and services you need.
Care Coordinators are regulated health professionals, with expertise in nursing, social work, occupational therapy, physiotherapy or speech therapy, who work directly with patients in hospitals, doctor's offices, communities, schools and in patients' homes. At your first home visit (or shortly afterwards), your Care Coordinator will:
- Learn more about you and your situation, to better understand your needs
- Answer your questions about services we provide
- Conduct a health-care assessment
- Develop a customized care plan that meets your specific needs
What else can I expect?
Once your care needs are determined, your Care Coordinator will discuss with you all the options available to meet your needs. As a team, we work together to ensure we connect to every part of the health-care system and we are focused on providing comprehensive care planning based on your individual needs.
Support can take a variety of forms. For instance, your Care Coordinator might suggest home care services provided by us or through service provider partners. Or, your Care Coordinator may refer you to additional community resources or government programs.
Whatever the support you require, your Care Coordinator can also provide help with completing forms, finding answers to questions and clarifying information.
Care at home
Care Coordinators are trained health-care professionals who can assess patients and provide advice on what services will best suit your needs. Your specific care plan is completely individual and geared towards achieving your optimal health and wellness at home and in your community.
Your home care team will identify the supplies you need to support your care and have them delivered to a depot location chosen by you at no cost to a depot in your region. Your Care coordinator or nurse will share the various options for Depot locations available to you and also provide information about options for direct delivery to your home.
