Hamilton Niagara Haldimand Brant Local Health Integration Network is committed to the delivery of safe, high quality patient care and services. If you or someone you know needs health care services at home, at school or in the community, we can help. We can also provide helpful information about supported living programs and long-term care options.
HNHB works with people of all ages to ensure they can make informed choices about their care, when and where they need it. We can also have useful information about local community support service agencies and can link you with providers to arrange services.
You can explore your care and support options through our website or contact us at 1-800-810-0000.
Recent Changes Related to Home and Community Care
On May 10, 2017, home and community care services and staff transferred from Community Care Access Centres (CCACs) to Local Health Integration Networks. Home and community care services are now provided through Ontario's 14 Local Health Integration Networks (LHINs). LHINs plan, integrate and fund local health care. We also deliver and coordinate home and community care.
As part of the Government of Ontario's Patients First: Action Plan for Health Care, this health system change was seamless for patients and home care clients to ensure continued delivery of uninterrupted care and support.
