A Message for our Patients and their Families

The Ministry of Health and Long-Term Care has released a discussion document: Patients First – A Proposal to Strengthen Patient-Centred Health Care in Ontario.


Please rest assured that your care plan and the service you receive are not changing as a result of proposed changes to CCAC's structure. The proposal is focused on greater integration and access to health services including primary care, public health and home and community care. We are pleased to see a continued emphasis on home and community care within our health care system. Under the proposal, Local Health Integration Networks (LHINs) would have an enhanced role and would govern and manage home and community care. CCAC employees who provide support to patients would be employed by LHINs and home care services would be provided by current service providers.


While the full details are yet to be decided, the government will consult on the paper over the next few months. We encourage patients and families to participate in discussions about the provision of health care Ontario. More information is available on the Ministry of Health and Long-Term Care website.


We wish to reassure our patients and families that our team continues to work with our partners to deliver safe, high-quality home and community care in accordance with patient care plans. HNHB CCAC will be an active participant in the discussions, and we are optimistic about how the Minister's vision will support continuous improvement in patient care.


Our team of highly-skilled, dedicated and experienced staff remains focused on providing quality care, in a fair and equitable manner, to all of the patients we serve.  Last year HNHB CCAC provided care and support to more than 82,000 individuals across our region. We will continue to work with the government and other health care providers to deliver home and community care through this transition and into the future.
