Anyone can call to find out about home and community care services. We have a responsive team of staff who are able to answer you questions and help you access the care you need.

All home care services and employees of the Hamilton Niagara Haldimand Brant Community Care Access Centre (CCAC) transferred to the Hamilton Niagara Haldimand Brant Local Health Integration Network (LHIN) on May 10th, 2017. This is part of Ontario's Patients First: Action Plan for Health Care which will increase access to care, reducing wait times and improving the patient experience.
You will continue to receive care and services without interruption and you will keep the same home care providers. Office locations remain the same. You can use the same phone numbers, email addresses and websites to get in touch with your providers and arrange your care.
As always, our priority is timely access to safe quality care for patients, their caregivers and families.
