People living in the Hamilton Niagara Haldimand Brant region can now connect with local
health resources tailored to their individual needs with a simple, free phone call.
The Hamilton Niagara Haldimand Brant Community Care Access Centre (HNHB CCAC) has launched My Health GPS – a health services information and referral line designed to
connect patients and caregivers to the exact types of local services they need. By calling 1-800-810-0000, anyone can have a conversation with an agent who is an expert in working with individuals to determine their precise needs. Every call is confidential and available with live translation in more than 200 languages.
Developed with input from patients, physicians, pharmacists and other health sector partners, My Health GPS provides access to information about local services, from the region's most detailed database of more than 4,000 community resources. Their agents can even help patients make appointments with certain agencies. Callers also receive a follow-up information package via email or regular mail that outlines all of the important information about the services recommended to them. At any time, patients can call back to ask questions or seek further assistance.
With My Health GPS, patients and caregivers do not have to navigate the health system alone. Help is available from 8:30 am to 8:30 pm, 365 days per year at 1-800-810-0000.
