About Share Care Council

​​​​​The Share Care Council is a 15-member committee consisting of patients, substitute decision-makers and family caregivers — volunteers committed to improving home and community care. Formed in 2014, it's a partnership that ensures a direct voice for patients and families to inform the development of new programs and services at the Mississauga Halton CCAC.

The Share Care Council is a direct means for the Mississauga Halton CCAC to learn what matters most to patients and their families. It is an authentic, engaging forum through which we gain tremendous insight to develop programming that meets the needs of patients and family members.

At the first two meetings, Share Care Council members:

  • Defined their Terms of Reference 
  • Determined scope of their work, goals and objectives
  • Identified their current priorities​ 
  • Provided feedback on what is needed so patients can be confident to leave hospital for home. This is part of our Strategic Plan, innovation initiative, Seamless Transitions: Hospital to Home (see video above)
  • Drafted a Patient and Caregiver Bill of Rights
