Carer Information Guide

Recent Changes Related to Home and Community Care

On May 31 2017, home and community care services and staff transferred from Community Care Access Centres (CCACs) to Local Health Integration Networks. Home and community care services are now provided through Ontario’s 14 Local Health Integration Networks (LHINs). LHINs plan, integrate and fund local health care. We also deliver and coordinate home and community care.

As part of the Government of Ontario’s Patients First: Action Plan for Health Care, this health system change was seamless in order to ensure uninterrupted care and support for patients and home care clients.
 

New ​Resource for Families and Caregivers

We know that family, friends, neighbours and other volunteers are important members in our patients' circles of care. Caring for elderly parents, family members or ailing friends requires time, attention and patience. It also requires people to take on new and sometimes challenging roles and learn quickly the best ways to support a loved one to remain at home.​

Our new carer guide, A Helping Hand While Caring for A Loved One, is full of information valuable to family/informal carers; it's a one-stop-shop resource where carers, at any chapter in their caregiving journey, can go to learn more about their vital role and how they can help their loved one live his or her best quality of life at home. The guide is relevant to all carers, even those living outside Mississauga Halton region, or those whose loved ones are not receiving support from us.
