The Health Council of Canada launched a nationwide video series, featuring innovative practices that are tackling waits for Canadian patients in various health care settings.
The CCAC Home First philosophy, which drives many services to help keep patients safe at home and out of hospital while they wait for long-term care, was showcased in one of the videos. Mississauga Halton CCAC CEO Caroline Brereton is featured in the video, along with Janet Parks, Director, Client Services.
"Home First is about providing the right care, in the right place, at the right time, via the right provider for seniors in our community,” says Caroline.
The video highlights how Home First helps manage Alternate Level of Care (ALC) rates in the province through intensive case management and homecare supports. Home First also helps patients move home safely, where they can take the time they need to apply for long-term care, remaining comfortable and surrounded by loved ones in their own home.
