On June 6, at the Ontario Association of Community Care Access Centres Awards of Excellence gala, the Mississauga Halton CCAC was recognized with the Sector Innovation Award for its Seamless Transitions: Hospital to Home initiative.
From brainstorming to design, implementation, testing and evaluation, Seamless Transitions re-imagined the patient experience as they move from hospital to home. Planning from the day of hospital admission through to discharge and the patient's first physician follow-up, the initiative's goal was to eliminate process duplications and reduce gaps in care and communication that can put patients at risk.
The Sector Innovation Award acknowledges the efforts of a team that has demonstrated outstanding, innovative leadership in developing and implementing initiatives driven from sector-shared purpose and priorities. The outcome data was a critical factor in our recognition. On average, patients were discharged nearly a day sooner, their satisfaction increased and the team reduced patient hospital readmissions by 52 per cent, 30 days post discharge in the pilot, on the Medicine unit.
