The North East CCAC is looking to establish formal partnership agreements with hospitals across the region that basically outline how the organizations will work together to identify and achieve mutually agreed upon performance targets. Our first agreement, with the North Bay Regional Health Centre (NBRHC), focused on patient transitions from hospital to home/community and achieved a 66% reduction in the length-of-stay of alternate level of care (ALC) patients.
As we continue to implement these and other initiatives contained in our improvement plan, I look forward to working with all health system partners to deliver on our CCAC’s key objectives:
- embracing the patient experience movement by improving patient engagement and choice,
- adopting innovative approaches to care coordination and service delivery,
- creating opportunities for electronic integration between health system partners and systems,
- enhancing communication to build trust and transparency with our partners, peers, and most importantly with patients and families across the North East.
