SUDBURY, ON – How do you help patients make the transition from the hospital back to the community, and provide the right care, at the right time, in the right place? What are the gaps between hospital and community-based care, and how can they be filled?
These questions and more will be explored at a free public forum being presented by Health Sciences North/Horizon Santé-Nord (HSN) and the North East Community Care Access Centre (NE CCAC).
“The successful transition of patients from the hospital back into the community is one of the most challenging but urgent health care issues were are facing in our city and northeastern Ontario, “says Dr. Denis Roy, HSN’s President and CEO. “It is going to take a total community effort to deal with this issue and that’s why this public forum is so valuable, so we can hear directly from the public on where the gaps are and how we can make the system better.”
The Care Transitions public forum is being held Thursday, October 30, at 6 pm at the Steelworkers Hall. The evening will also include a vendor’s expo of goods and services related to home care.
“To work together effectively and provide true patient-centred care, we have torn down the silos that used to exist in the health care system, integrated our efforts, and included the patient voice,” states Richard Joly, CEO, NE CCAC. “Our goal is to find the most appropriate care venue for each individual patient, whether that’s home, assisted living, a retirement home or long-term care facility.”
The Care Transitions public forum is being moderated by Steve Paikin, award-winning journalist and author, best-known as host of TVO’s flagship current affairs program “The Agenda.” The forum will feature a panel of health care leaders, and live participation from the audience.
