Access To Care​​

In February 2016 Rapid Response Nurses began providing regional seven-day coverage for patients in Timmins, Sault Ste. Marie, Elliot Lake, North Bay and Sudbury – a signi​ficant step forward for patient care and care transitions.

​​ ​The new schedule ensured that regardless of when patients were discharged from hospital, they received a phone assessment within 24 hours, followed by a home visit by the local Rapid Response nurse to address any immediate concerns.

 



Through a great team effort among Care Coordinators, Team Assistants and therapists, the North East CCAC was successful in achieving its 
community referral to first service goals. The CCAC is now well on track to achieve the Province's 21-day wait time target – no small feat for an underserviced and geographically vast area. 



While patients in the North East always had the option to receive CCAC services in French, in 2015, the CCAC received its long-awaited official designation under the French Language Services Act. The FLS designation was a sign of the organization's enhanced commitment to ​​ensuring patients fully understood their individual care plans including the services that were being offered and the expected outcomes.
