The Ministry of Health, Ministry of Long-Term Care and Home and Community Care Support Services are each responsible for developing and fulfilling clear and achievable performance obligations, and to identify any risks to performance. All parties are committed to working collaboratively by establishing clear lines of communication and responsibility, and by working diligently to resolve issues in a proactive and timely manner.

Home and Community Care Support Services Priorities Letter
The priorities letter
is part of the Ministry of Health’s accountability framework for Home and
Community Care Support Services. One of the foundational initiatives is
modernizing home and community care services to improve patient access to care
and to enable successful integration of these services within the Ontario
Health Teams. Key priorities and
expectations for the 2021-2022 fiscal year include: competitiveness,
sustainability and expenditure management; transparency and accountability;
risk management; workforce management; data collection; digital delivery and
customer service; diversity and inclusion; and COVID-19 recovery. To view the

A combined Home and Community Care Support Services 2021-22 Interim Business Planning document detailing how the priorities set out in the 2021-22 Interim Priorities Letter will be operationalized is currently under development and will also be posted when available.
Ontario is improving the health care system to give patients better connected care, focused on their needs.
On June 6, 2019 the Government of Ontario proclaimed the Connecting Care Act, 2019 to formally establish Ontario Health as a Crown agency in support of the eventual consolidation of LHINs and six provincial agencies into Ontario Health.
On April 1, 2021, non-patient care functions from the LHINs transferred into Ontario Health. Patient care functions including home and community care, long-term care home placement and key supporting functions remain in place under a new name to reflect a re-focused mandate – Home and Community Care Support Services.
A cross-appointed Home and Community Care Support Services Board of Directors was put into place, effective July 1, 2021. This highly skilled and diverse group oversees all Ontario's Home and Community Care Support Services organizations and supports the continued provision of high-quality home care services with a cohesive provincial vision in support of the government's plan to modernize home and community care.

You will continue to have access to the care you need and the health care providers you have built relationships with. Home care continues as before and there are no changes to long-term care home placement.
Our patient services and contacts will remain unchanged until further notice.
