You Can Help to Inform the Future of Home and Community Care in Ontario


The Ministry of Health and Long-Term Care (MOHLTC) is consulting with patients and caregivers to inform the future of home and community care. The Ministry is asking Ontarians to provide input on a Statement of Values and help inform what the province's home and community care sector should strive to deliver. Go here to complete the survey and provide your input on the future of home and community care in Ontario before September 12, 2016.


Home and community care services support people of all ages who require care in their home, at school or in the community. The Statement of Values survey is part of the Ontario government's commitment to put patients first by consulting with patients and caregivers to help inform the future of home and community care.

The purpose of the Statement of Values is to:

  • Give patients, caregivers and families a greater voice in the care process.
  • Articulate shared values to influence and guide the way home and community care is delivered by service providers.

Guide system-wide improvement by providing a statement to guide all care partners in the system.

The Statement of Values will become a checkpoint for policy and decision makers, care providers, and government to ensure that all policies and programs related to home and community care embody these values. Moving forward, the government will work with Local Health Integration Networks (LHINs), Community Care Access Centres (CCACs), sector leaders, service providers, patients and caregivers to embed the Statement into their everyday business – which is the delivery of home and community care that has patients and caregivers at its centre. The Statement of Values will also help set priorities, build a unified sector through a shared understanding of what patients and caregivers value, and describe hopes for the future.
