Job Summary:


Under the direction of the Senior Director, Community Care, the Director, Community Care is responsible for ensuring the provision of consistent services to clients and their families across North Western Ontario. The Director, Community Care is a liaison with other departments, programs and agencies that promote and support community care, both internal and external to the North West Community Care Access Centre. The Director, Community Care is an active member of the management team who supports the corporate strategic/business plan.


  • Bachelor's Degree in a Health Related discipline (e.g. Nursing, Occupational Therapy, Physiotherapy, Social Work, Speech-language Pathology) a preferred asset
  • Minimum five (5) years' experience in a leadership role
  • Management experience responsible for a large division in a unionized environment with direct experience with change management
  • Knowledge of legislation governing health care activities in Ontario
  • Excellent knowledge of community resources and roles of health professionals; excellent assessment, decision making, priority setting skills, and the ability to demonstrate effective working relationships
  • Experience in Risk Management
  • Experience in Quality Management
  • Experience with computers in a Windows environment (e.g. Microsoft Office and database programs)
  • Demonstrates a commitment to personal and client safety
  • Ability to manage multiple tasks within tight timelines
  • Excellent problem-solving and critical thinking skills
  • Excellent organizational and time management skills
  • Excellent communication and engagement skills
  • Experience in a team environment
  • French language is an asset

Also required:

  • Proof of registration in applicable regulatory College
  • Proof of a valid driver's license
  • Personal transportation
