Job Summary:

One Temporary Full Time Position

(1-year contract position with potential to extend to longer term)

POSITION DESCRIPTION:

Reporting to the Community Care Manager, the Telehomecare Engagement and Implementation Lead will be responsible for leading the adoption and integration of the Telehomecare Program across the North West CCAC to meet program goals and objectives across the NWCCAC catchment area.

QUALIFICATIONS:

  • Proven experience and background in Marketing
  • Baccalaureate Degree in a health related discipline is considered an asset
  • Experience in Chronic Disease Prevention and Management of COPD and CHF is a strong asset
  • Superior and proven skills with emerging technology and the use of innovative technologies (tablets, e-referral, e-health coaching) in healthcare and in homecare
  • Excellent interpersonal skills with experience in engaging health system leaders
  • Self-starter, with outstanding customer service
  • Effective communicator, able to communicate effectively and concisely, both orally and in writing, including effective presentation skills
  • Demonstrated strength building relationships, particularly with health care partners and Family Health Teams
  • Excellent understanding of Ontario's healthcare system and healthcare administration
  • Ideally, at least two years of  experience in community health or related field
  • Excellent problem-solving skills, negotiation, issues management, organizational, project management, decision-making and analytical skills
  • Proven experience in developing, documenting and implementing best practices
  • Proven track record of being able to deliver on organizational targets
  • Actively engages in the identification of leading practice and evidence based data to support planning
  • Ability to influence change in a dynamic established environment
  • Comprehensively use software including database, spreadsheet, word-processing, presentation, electronic mail and calendaring
    Also required:
  • Will be required to travel within the NWCCAC to deliver Telehomecare program
  • Proof of a valid driver's license
  • Personal transportation
