Telehomecare is a program that provides people with conditions such as congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD), with weekly health coaching and daily monitoring of vital signs in their own homes.
Telehomecare is supported by the Ontario Telemedicine Network (OTN). This program is designed to help patients achieve the best possible quality of life by learning to manage their condition.
How does this program work?
There are two program streams that support coaching and monitoring patients living with congestive heart failure (CHF) and/or chronic obstructive pulmonary disease (COPD).
If you are part of either program, you will receive guidance on checking your own blood pressure, weight, heart rate and pulse. Through a touch-screen tablet provided by the program, you can then send your results, along with the answers to a few simple questions, to your Telehomecare nurse. If your readings fall outside of your normal range, your nurse is alerted and can respond as needed.
The Coaching Stream is up to a six month program for patients living with mild to moderate CHF and/or COPD. In addition to remote monitoring, your nurse provides weekly health coaching geared to your needs. This may include guidance regarding diet, exercise and other factors for optimal health.
The Monitoring Stream is a one to three month program to support patients living with CHF and/or COPD that are going back home after a hospital stay and require additional support to manage well at home with medication management and education on sign and symptom management.
The goal of Telehomecare Nursing, is to help a patient and their caregivers feel more confident in their management of chronic conditions.
