How CCACs Care: An update on quality improvement for patients shares the most up-to-date information available on CCACs' progress on key priorities for improving quality, the patient experience and patient safety in home care across Ontario. This web-based update reflects a sector-wide commitment to continuous quality improvement and presents information showing year-over-year progress on performance measures.

Health Quality Ontario (HQO) home care reporting
As of December 31, 2012, all of Ontario's 14 CCACs achieved third-party accreditation with outstanding results. This achievement demonstrates another way in which CCACs are working together proactively to meet the goals of the
Excellent Care for All Act. Although CCACs can be accredited through different organizations –
Accreditation Canada, or the
Commission on Accreditation of Rehabilitation Facilities (CARF) International, the goal is the same: evaluating the quality of the care and services provided to patients and measuring quality improvement initiatives against national standards for the delivery of quality health care.

Quality Improvement Plans
The goal of all CCACs is to deliver high-quality, integrated care to our patients, as is reflected in our vision – Outstanding care, every person, every day. Additionally, CCACs are key collaborators in the health care system, supporting system integration with our health care partners in order to achieve the objective of delivering the best possible care to Ontarians.
In keeping with this commitment, all 14 CCACs have developed Quality Improvement Plans (QIP) aligned with the key principles found in the
Excellent Care For All Act (ECFAA) and as directed by the Ministry of Health and Long-Term Care. The QIP is one tool that helps to define organizational priorities focused on delivering patients the best care possible through quality initiatives and continuous quality improvement. Under this process, the QIP is also central to progressive health system transformation by ensuring all health sectors partner for quality, patient-centred care.

Across the province, we are introducing an outcome-based approach to care as a new way of providing home and community care. Through a team-based approach, outcome-based care uses best practices to achieve positive results for people with similar care needs, such as wound care or better mobility following hip or knee surgery.
