Since the inception of the
provincial Health Care Connect (HCC) Program in 2009, the North Simcoe Muskoka (NSM) CCAC’s team of Care Connectors has been helping individuals and families without a regular health care provider get connected to a doctor or nurse practitioner. To date, the team has successfully matched over 30,000 people to a primary care provider in NSM.
In collaboration with community partners, residents are
experiencing better access to resources that promote health, prevent illness and support chronic disease management. In Orillia and surrounding area, Health Care Connect partners are supporting better access to care for patients when and where they need it.

"For the past 3 years, the Orillia & Area Physician Recruitment & Retention Committee
(OAPRRC) has initiated and hosted 10 community outreach events each year, in
partnership with Health Care Connect. These community activities serve to assist residents in their search for a local family physician in Orillia and area. This has been a very cooperative and fruitful endeavor. It is estimated that conservatively we have assisted well over
3,000 local residents as a result of these collaborative outreach events."

Success by the Numbers
- 30,559 The number of patients the NSM CCAC Health Care Connect team has matched to a primary care provider since February 2009.
- 67% Patients connected to a primary care provider within 10km of their home.
- Of patients connected to date, 2,792 were identified as complex vulnerable.

To access the Health Care Connect service, a patient must be currently without a primary care provider, and have a valid Ontario Health Card.
