NSM CCAC achieves Exemplary Standing

Accreditation Canada is an independent, not-for-profit agencyACSealAwES.PNG renowned for their work in setting standards for quality and safety in healthcare, and accredits health and social service organizations nationally and internationally. Their mission – Better Quality. Better Health.

The North Simcoe Muskoka Community Care Access Centre (NSM CCAC) voluntarily participated in Accreditation Canada's program in 2011 and achieved Exemplary Standing - the highest level of award conferred by Accreditation Canada.  NSM CCAC values the contribution accreditation makes to quality improvement, risk management and accountability.

NSM CCAC again committed to participating in the accreditation program this November, and was evaluated on compliance with required organizational practices (ROPs) and standards that contribute to high quality, safe, and effectively managed care.  As a result, NSM CCAC celebrates achieving Full Accreditation with Exemplary Standing in their second, consecutive survey by Accreditation Canada.

"Year after year we have continued to build on a strong foundation in delivering on our commitment to work hand-in-hand with patients, their caregivers and families to make our system better, improve the overall health care experience, and achieve the best value from our finite health resources.  Achieving Exemplary Standing again showcases how quality improvement planning enables enhanced coordination and integration of services for improved outcomes, and puts a spotlight on leading practices in our day-to-day efforts.  This outstanding achievement is a reflection of the tremendous efforts by all and is a great external validation of our culture of quality improvement and our commitment to safe, quality care."

Colleen Geiger, Board Chair, NSM CCAC

"NSM CCAC is committed to our vision:  Outstanding care - every person, every day.  Our Board and staff have once again demonstrated their commitment to excellence in quality and safety for our patients and their families.  This is a proud accomplishment for everyone on our team."

NSM CCAC is one of 14 publicly-funded agencies in Ontario, bringing care into homes, schools and community clinics for people of all ages.  Every day the care team helps people understand care options, get information they need to make informed choices and coordinates care delivery for patients.

In 2014-15, the North Simcoe Muskoka CCAC brought more care into homes across our communities than ever before.  Nearly 25,000 people got the care they needed by the CCAC care team, and of these people, 14,500 were seniors 65 years of age and older.  CCAC care experienced in the home touched 2,400 people on an average day and involved care coordination, nursing, personal support, physiotherapy, occupational therapy, respiratory therapy, speech and language therapy, and social work from our dedicated team of health professionals.

As the single point of access to supportive care settings, like convalescent care, assisted living and long-term care, NSM CCAC helps transition loved ones to new care settings when needed.  Every person's situation is unique, and plans of care are co-created with patients, families and partners to reflect needs, values, safety and quality.  Over 97% of patients and caregivers surveyed indicated that they would recommend NSM CCAC to family and friends.
