North Simcoe Muskoka CCAC is bringing care home through the expansion of the regional Telehomecare Program for patients. This increased investment to in-home and community care services supports putting Patients First, while reducing avoidable hospital readmissions.
With new funding, Telehomecare is expanding to enhance the program offering with an online monitoring component for patients with complex health needs.
Telehomecare links patients with chronic conditions to Registered Nurses who provide remote monitoring and regular health coaching. Patients become partners in their own care - right in their own homes.
Telehomecare complements the care already provided to patients living with Chronic Obstructive Pulmonary Disease (COPD) and Congestive Heart Failure (CHF), with a focus on:
learning self-management skills,
early identification of exacerbations, and
- avoiding unnecessary trips to the hospital emergency room.
"Telehomecare has taught met to be more focused and mindful of my condition."
